Creating a Multiple-Choice Test Template:

  1. Create a new blank word document
  2. Turn on the Forms toolbar - View, Toolbars, Forms
  3. Center align the first line
  4. Change the font to Arial size 14
  5. Type a test heading with Course Name/Number - ex: History 1121
  6. Press the enter key
  7. Mouse click the "Text Form Field" icon ab| on the Forms toolbar
    This is where you will enter the test name
  8. Press the enter key
  9. Click the "Text Form Field" icon ab| on the Forms toolbar
    This is where you will enter the test date
  10. Click the "Form Field Options" icon (hand holding form) on the Forms toolbar
  11. Select 'Date' as the type
  12. Select a Date format of your choice
  13. Press the enter key twice
  14. Select the "Left alignment" icon on the formatting toolbar
  15. Type "1." and press the tab key
  16. Insert a Text Form Field from the Forms toolbar
    This is were you will type your first question
  17. Press the enter key twice
  18. Select the "increase indent" icon (blue right arrow) on the formatting toolbar
  19. Type "A." and press the tab key
  20. Insert a Text Form Field from the Forms toolbar
    This is were you will type your first answer
  21. Press the enter key
  22. Type "B." and press the tab key
  23. Insert a Text Form Field from the Forms toolbar
    This is were you will type your second answer
  24. Enter your C & D answer fields
  25. Press the enter key twice
  26. Press the "decrease indent" icon (blue left arrow)
  27. OPTIONAL
    If you want your answers listed in two columns
    a. Select you answers
    b. Select "Format" from the menu
    c. Select "Columns"
    d. Select "2"
  28. Select and copy your entire question one
  29. Paste the question at the paragraph maker past questions one
  30. Continue pasting for the number of questions that you need on your template
  31. Renumber the questions
  32. Select "Tools" from the menu, then "Protect Document"
  33. Select "Forms" in the dialog box and press "OK"
  34. Select "File" from the menu and then "Save as"
  35. In the "Save as type" box, select "Template" then give the file a descriptive name
    Notice that the destination folder has changed to "Templates". Word will save
    the template in the proper template folder for you.
  36. Close the file
  37. Select "File" from the menu, then "New"
    Your template will be listed under the General Tab
  38. Double-click on your template
    This will open a new blank document based on your template.
  39. Type your fist question and tab to the first answer and type. You will use your tab key,
    shift + tab, and arrow keys to navigate between your fields
  40. Select "Tools" from the menu, then "Unprotect document" to edit outside of the form fields.
